Risk Acknowledgment
风险承认Recognition that indoor open-flame catering carries inherent risks — smoke, heat, and the possibility of fire-alarm activation under modern sprinkler systems.
"Bilingual EN / 中文 · Attorney-reviewed · E-SIGN and UETA-compliant."
Indoor live-flame teppanyaki is a real culinary art and a real liability. Before Chef BENIHAHA arrives at your venue, every host signs the same attorney-reviewed bilingual agreement protecting your property, our team, and your guests. The signing flow takes about three minutes.
Recognition that indoor open-flame catering carries inherent risks — smoke, heat, and the possibility of fire-alarm activation under modern sprinkler systems.
The host confirms ceiling clearance ≥ 8 ft, no flammable materials within 5 ft of the cooking station, and adequate ventilation.
Building-wide alarm activation is a structural property condition, not provider fault. The host coordinates with building management in advance.
Minor surface markings within a 10-foot radius of the cooking station (grease micro-splatter, radiant heat micro-cracking on rare stone) are non-compensable.
Host must disclose all guest allergies 48 hours in advance. Chef BENIHAHA's kitchen is ServSafe-certified and follows allergen-isolation protocols.
Commercial General Liability $1,000,000 per occurrence. Certificate of Insurance available 7 days before any event on request.
Optional release for chef's marketing photos of the cooking demo. Guest faces are never the focus. Easy opt-out on-site before service begins.
Weather or safety-driven cancellations from our side are rescheduled at no charge or refunded. Client-side cancellations follow the published policy.
"These three items carry the most direct risk to your property and your guests. You'll initial each box in the signing flow."

Yes. The signing flow is built around the federal E-SIGN Act (2000) and New York's UETA (2002). Your electronic signature has the same legal effect as a handwritten signature, with full audit trail (timestamp, signing ID, IP-verified delivery).
Yes. The document is rendered in parallel English and 中文. The English text is the controlling text in any conflict of interpretation — standard for bilingual contracts — but every clause has a side-by-side Chinese translation for clarity.
Yes. We can email a clean preview PDF to your legal or HR team before issuing the signing link. Mention this in the request form's phone field or email Info@ezhomakase.com.
For high-rise apartments and condos, building management often requires (a) a copy of our $1M Certificate of Insurance and (b) their own confirmation form. Forward the COI to them at least 7 days before the event. We deliver the COI on request.
The countersigned PDF is filed to our records, a copy is emailed to you instantly, and our event coordinator receives a notification to begin event logistics — menu confirmation, allergy intake, parking, kitchen access, and pre-event call.
